Orthognathic Surgery Cases
Some patients require orthognathic surgery (jaw surgery by an oral surgeon) to correct abnormal jaw bone growth. All of these patients also require orthodontic treatment (braces) to coordinate the arches. Usually we treat with braces to align the teeth for about 18 months before the surgical procedure. When the teeth are straight and the arches are coordinated, the surgeon completes the orthognathic surgery with the braces still in place; and after the patient heals the case is completed with the braces over a six to ten month period. Surgery patients are retained in the usual manner.
Surgically Moving The Lower Jaw Back And The Upper Jaw Forward
This patient had the upper jaw advanced 5mm with a 3mm impaction on the right and a 5mm impaction on the left. The left was impacted more than the right to correct the cant of the smile. The lower jaw was moved back 4mm with bilateral split oseotomies and a genioplasty was also performed. The chin was down grafted 2mm and advanced 8mm. Bone was harvested from the left anterior hip for the grafts.

Advancing the Lower Jaw Surgically
This patient had braces to level and align the teeth followed by surgery to advance the lower jaw forward. The chin was also advanced surgically.
